• Student’s signature on application
• Parent(s) signature on application
• Proof of FAFSA (Confirmation email is sufficient)
• Proof of Homestead Exemption (Available at Lake County Auditor’s Office 219-755-
• 3120. Must be date-stamped with year of original purchase.)
• Official Transcripts (Final transcript – available after graduation)
• Proof of SAT/ ACT Scores
• Acceptance letter to an accredited Indiana based College/University
